Jack O. Smalley, DDS
Dr. Jack Smalley graduated from New Mexico State University before attending dental school at the University of Missouri – Kansas City, from which he graduated with his Doctor of Dental Surgery (DDS) in 1993. He completed his General Practice Residency in 1995. He pursues non-stop continuing education through both classes and study groups, and is a member of the American Dental Association. Dr. Smalley has practiced in the Farmington, NM, area since 1997. As a dentist, his goal is to create and maintain healthy, happy smiles for people of all ages.
Dr. Smalley is a native New Mexican, and settled in Farmington because he likes the people and the diverse activities in the 4 Corners region. It has been his honor to serve the people in this area for more than 20 years. Outside of the office, Dr. Smalley enjoys spending time with family, skiing, hunting, fishing, and golf.
Dr. Reese Foutz
Born and raised in Kirtland, New Mexico, Dr. Reese Foutz is proud to return home to serve the community that shaped him. A graduate of Kirtland Central High School, Dr. Foutz first pursued a degree in Petroleum and Natural Gas Engineering from New Mexico Tech. His early career took him to the Gulf of Mexico, working on offshore oil rigs – but it was there he realized something was missing. Driven by a deeper calling to help people more directly, he returned to school to pursue dentistry and earned his Doctor of Dental Medicine degree from Midwestern University in Glendale, Arizona.
Dr. Foutz brings not only technical skill but also compassion and understanding to every patient interaction. Growing up in the Four Corners area, he recognizes the unique challenges many face in accessing quality dental care. He is passionate about creating a welcoming environment where patients feel comfortable, respected and truly heard. Whether treating a toothache or helping someone regain confidence in their smile, Dr. Foutz is dedicated to improving the health and well-being of his hometown community.
Outside the office, Dr. Foutz enjoys spending time with his wife, Chera, and their two dogs, Bandit and Coop. He is an avid golfer, loves playing basketball and takes every opportunity to enjoy the outdoors.
DeAnza Mead
DeAnza has been in the dental field since 1995, and has been with Dr. Smalley’s office since 2004. She strives to improve our office and educate our patients. Her favorite part of our practice is our wonderful team that works together to give patients a best possible dental experience.
DeAnza was born and raised in Farmington, and enjoys being outdoors, yoga, and spending time with her family.
